BCI and brain-machine interfaces for communication restoration in paralysis

• Intracortical speech neuroprosthesis (BrainGate2; NEJM 2024) restored conversational communication in a man with ALS and severe dysarthria using 256 electrodes in left ventral precentral gyrus.¹
• After ~30 minutes of calibration, day-1 accuracy reached 99.6% on a 50-word vocabulary; with further training, 97.5% accuracy on a 125k-word vocabulary at ~32 words/min over 8.4 months.¹
• Earlier Nature work (Metzger, Chang et al., 2023) demonstrated high-performance speech decoding and avatar control from cortical activity, establishing the clinical trajectory for speech BCIs in paralysis.²
