- Motif’s lead product is the DOT (Digitally programmable Over-brain Therapeutic), a millimeter-scale wireless brain stimulator roughly 9 mm across (described as blueberry- or pea-sized) that sits in the skull above the cortex without penetrating the dura or contacting brain tissue.23
- The device is battery-free, powered by magnetoelectric wireless power-transfer technology invented and patented at Rice University and exclusively licensed by Motif, enabling extreme miniaturization by removing the battery (normally the largest component of implantable stimulators).23

- The trial (NCT07594483) is a prospective, multi-center, single-arm, open-label early feasibility study with an estimated enrollment of 20 participants, an estimated start of July 2026 and completion around December 2027.5
- The implant is delivered through a burr hole (~14 mm) over the left dorsolateral prefrontal cortex (DLPFC) in a ~20-minute outpatient procedure; eligibility requires skull thickness between 5.5 mm and 15.5 mm over the target.54
- The trial’s primary efficacy outcome is change in the Montgomery-Åsberg Depression Rating Scale (MADRS, range 0-60) from post-treatment through 12-month follow-up, alongside tracking of stimulation-induced side effects.5
- The system is branded the XCS (eXternally powered Cortical Stimulation) System, positioned as a middle rung between non-invasive TMS and invasive DBS, delivering TMS-grade stimulation at home via a wearable transmitter (“hat”).24
- Preclinical and early human work — a collaboration with Rice University, Baylor College of Medicine and UTHealth Houston, with intraoperative studies at Baylor St. Luke’s Medical Center — showed the glass-sealed implant stimulated the brain through the dura and operated reliably in freely behaving pigs over roughly one month.63
- Beyond depression, Motif has said it is exploring the DOT for bipolar disorder, OCD, Alzheimer’s disease and substance use disorder; the company is led by CEO Jacob Robinson.27
