- Cognito Therapeutics and West Virginia University’s Rockefeller Neuroscience Institute (RNI) announced a “Brain Health Collaboratory” partnership (November 2025), building a rural, neurotech-enabled learning healthcare system initially focused on Alzheimer’s disease and co-funded by both organizations.2
- Cognito Therapeutics was founded in 2016 by neuroscientists Ed Boyden and Li-Huei Tsai and has raised over $200M in private capital; its Spectris headset delivers combined 40 Hz auditory and light (gamma) sensory stimulation for one hour per day at home.2
- Spectris earned FDA Breakthrough Device designation (2020–2021), and Cognito’s pivotal HOPE trial (NCT05637801) is fully enrolled with roughly 670–673 patients, with study completion anticipated in spring 2026.234
- HOPE is a randomized, double-blind, sham-controlled study (1:1 active:sham) across up to ~70 US clinical sites, using the device 60 minutes daily for approximately 12 months followed by a 12-month open-label extension, in mild-to-moderate AD (MMSE 15–28).34
- The primary endpoint is the Integrated Alzheimer’s Disease MMSE + ADCS-ADL Rating Scale (iADMARS), a combined measure of cognition (MMSE) and daily function (ADCS-ADL) at 12 months.4
- The earlier OVERTURE feasibility study (NCT03556280) reported a 76% reduction in cognitive decline (MMSE), a 77% reduction in functional decline (ADCS-ADL), and a 69% reduction in whole-brain atrophy over 6 months by MRI.35
- RNI, described as an eight-year-old unit integrating neurology, neurosurgery, psychiatry, radiology, and rehab, cares for about 300,000 patients across a rural footprint (West Virginia, SW Pennsylvania, Eastern Ohio, Western Maryland), including roughly 10,000 Alzheimer’s and 9,000 Parkinson’s patients annually across 47 practice locations.2
- RNI, led by neurosurgeon Ali Rezai, also pioneered focused-ultrasound blood-brain-barrier opening for Alzheimer’s: a first-in-human NEJM study paired MRI-guided focused ultrasound (an Insightec helmet with more than 1,000 transducers) with the anti-amyloid antibody aducanumab to accelerate amyloid-beta clearance (about 32% faster), with the barrier closing within 24–48 hours and no serious adverse events reported.67
